VISA INSTRUCTION GUIDE FOR CUBA
Tourist Visa for US Citizens (Tourist Card) - Please send the following:
1) Valid, signed original passport- must be valid for at least 6 months beyond your intended stay in Cuba.
2) Completed Visa Application Form.
3) One recent passport size photograph, in color.
4) Flight Itinerary showing onward and return flight.
Business Visa for US Citizens-
Personal Appearance is required. Please contact the Embassy of Cuba in Washington, D.C. for additional details at 202-797-8518.

WPVS fee: $100
