VISA INSTRUCTION GUIDE FOR AZERBAIJAN

TOURIST VISA - Please send the following:

1. Valid and signed passport must have at least 6 months validity from expiration date of passport.

2. One completed application form. All questions answered and signed.

3.Two (2) passport size photo, color.

4.Tourist voucher from hotel in Azerbaijan that lists the hotel confirmation number

5. Copy of airline tickets, cruise tickets or travel agent itinerary.

BUSINESS VISA - Please send the following:

1. Valid and signed passport must have at least 6 months validity from expiration date of passport.

2. One completed application form.All questions answered and signed.

3.Two (2) passport size photo, color.

4. Copy of government approved invitation from the Azerbaijan Ministry of Foreign Affairs. This must contain the government seal and reference numbers. Invite will specify single, double or multi entry. Multi entry requires HIV-test Medical Certificate.

5. Copy of your airline ticket or travel agent itinerary.
