INSTRUCTION GUIDE FOR U.S. PASSPORT RENEWALS
1) Passport:
Must submit your most recently issued passport. The passport must have been issued within the last 15 years. If issued over 15 years, you will not qualify for a passport renewal, and must apply for a First Time Passport.
2) Photograph:
You must include 1 recent color photograph, 2 inches by 2 inches printed in photo quality paper. Effective immediately: Applicants are no longer allowed to wear glasses in their passport-style photograph being submitted with Passport Renewal request.
3) Letter of Authorization:
Two Letter of Authorization must contain ORIGINAL signature. Fax and scanned copies will not be acceptable. No mistakes on document will be allowed. If document is completed incorrectly, Department of State will place Passport Renewal request on hold.
4) DS-82 form:
DS-82 form will be the Passport Renewal Application form. You must complete application online, and once finalized, you will be able to print out application form. Keep in mind, Hand-Written forms will not be accepted by Department of State. Once Application is printed, it must contain original signature from Applicant.
5) Name Change:
If your passport was issued more than 1 year ago and you require a name change, you must send original documentation by marriage or court order. Photocopies and notarized documents will not be accepted.
6) Passport Card:
If you will be requesting a Passport Card, keep in mind, the Passport Card cannot be expedited and will be processed in about 2-3 weeks by the Department of State. There is an additional cost of $30.00 for the Passport Card. Once completed, it will be mailed to you by First Class mail from the Department of State.
7) WPVS Passport Order form and Credit Card Form:
WPVS Passport Order form must be included in your request. You can find document under "Document Links" on the right side. Applicants submitting payments via Credit card must complete Credit Card form and also include with your request.

Passport Renewal (Passport book and card): $220.00
Fee may be billed to credit card if payment is made via credit card.
