VISA INSTRUCTION GUIDE FOR BELARUS
TOURIST VISA - Please send the following:
1. U.S. Passport, signed and valid for at least 6 months.
2. One completed application form. All questions answered and signed. The form must be filled out completely, no blank spaces must be left. If a question does not apply, please type "N/A". If the answer is none, write "NONE". Incomplete forms will be returned to the applicants UNPROCESSED.
3. One (1) passport size photo, color.
4. An original or a copy of a letter of invitation from a Belarusian Travel Agency. Hosting organization should be registered as a tourist company by the Belarusian authorities.
5. Proof of International Medical Insurance
A letter from your insurance company (original or copy) is required. This letter should contain the following information.
1) Name, address and phone number of the Insurance Company.
2) Full name of policy holder.
3) Insurance territorial coverage (should be valid in Belarus or worldwide).
4) Insurance term of validity (should be valid through the period of travel or temporary residence in Belarus).
5) Amount of medical coverage (should not be less than $13.500.00).
A photocopy of the insurance card will not be sufficient enough to meet the above requirements.
Please Note: The ORIGINAL medical insurance documentation/letter must be carried by the traveler during travel to Belars
6. Copy of airline tickets or travel agent itinerary.
7. Citizens of the US, Canada, European Union states and some other countries (please, check at the Embassy), who applies for private visa to the Republic of Belarus valid through 30 days or less do not need to provide any letters of invitation. Still, the inviting person's first and last names and home address must be indicated in the visa application form.
BUSINESS VISA - Please send the following:
1. U.S. Passport, signed and valid for at least 6 months.

4. Copy of your letter of invitation from your host/sponsor in Belarus. Invitation must be approved by the Ministry of Foreign Affairs in Belarus and telexed to the embassy in the U.S.

6. Business cover letter from your company stating purpose of travel, dates of travel, name and contact of the company in Belarus and financial guarantee for all your expenses.
7. Copy of your airline ticket or travel agent itinerary.
